The Community Reentry Services Program take a whole-person approach to case management, connecting program participants with services and resources to improve the individual’s overalll wellness, and reduce the risk of re-offending. Case managers and participants discuss each individual’s history and current needs, and together they create a plan to work on the individual’s goals.This program can assist with or help participants access: – GED/high school diploma – College or certificate programs – Vocational training programs – Social Services – Housing – Basic Needs (food, clothing, ID, etc.) – Job readiness skill building, job search, and employment – Family Support Services – Sunstance abuse services & mental health services – Healthcare
