Program provides companion counseling, friendly visiting, telephone reassurance, medical transportation, light housekeeping, meal preparation, errand running, short term respite, caregiver support and referrals to community resources for persons with chronic health problems, especially senior citizens, disabled susceptible to abuse, neglect or exploitation, and their caregiving families.
