Christiana Care Health System: Transition Care Practice

Christiana Care Health System (CCHS) offers several transition practices that are collaborative in nature between duPont Hospital for Children, Christiana Care Health System and the University of Delaware, Center for Disabilities Studies. At A.I. du Pont Hospital for Children (DHC) there is a consultative transition of care service and transition division to prepare patients and families to enter the adult health care system. As well there is a clinic for survivors of pediatric cancer, childhood HIV, pediatric cystic fibrosis, congenital heart disease and cochlear implants. At Christiana Care Health System (CCHS), there is a primary care transition care practice as well as congenital heart disease, cystic fibrosis, HIV and survivor of childhood cancer clinics. Residents learn to manage complicated patients with multiple medical and social issues including technology dependency, communication difficulties, tubes and shunts in the inpatient and outpatient setting.
