Delaware Hospice provides the highest quality hospice and healthcare services, and serves as a trusted community partner in end-of-life- education and support. We provide hospice or palliative care in the family’s home, assisted livig facility, and nursing home. The Delaware Hospice Center offers a higher level of specialized, 24 – hour care to hospice patients who require symptom management. The Care Team consists of a nurse, nurse’s aid, social workers, chaplain, and volunteer in conjunction with the primary physician.
