The Healthy Families Program at the LACC provides community members with navigation of the health care system. Clients will receive a health assessment at intake. Services provided include help identifying doctors/healthcare practitioners/specialists, understanding medical bills and arranging payments for them, screening for cancer, screening for diabetes, and signing clients up for programs like Screening for Life (SFL) and Health Care Connection (HCC).
